AOD Counselor/Care Manager (Personal Services)



CLASSIFCATION SUMMARY:

Volunteers of America is looking for a dynamic person to help create change in our communities through our Renaissance Outpatient Services Program. Renaissance Outpatient Services is a multifaceted program that provides services tailored to our clients' needs. The program is flexible and adaptive, offering outpatient treatment, case management, supportive services, benefit enrollment assistance, and supportive housing services. In addition, our clients are co-enrolled in our Enhanced Care Management program. We are seeking an AOD Counselor/Care Manager to join our team, help develop our program and services, and deliver meaningful support. Monday through Friday shifts.

ESSENTIAL DUTIES AND RESPONSIBILITES:

  • Utilizing information gathered at intake and through ongoing assessment, collaborate with the client and the treatment team to determine needs and create individualized case management service plans for clients.  
  • Completing initial and updated treatment plan within the specified time frames for all clients on your caseload. 
  • Comprehensive assessment and periodic reassessment of individual needs to determine the need for the continuation of services; revision of client treatment plan when necessary. 
  • Actively help clients to apply for and maintain health and public benefits (i.e., Medi-Cal, Healthy San Diego, Perinatal, Housing, etc.). 
  • Link clients to community resources and services (i.e., transportation, food and clothing assistance, family planning services, legal assistance, educational resources and job training, exc.). 
  • Provide direct services to clients, including one-on-one meetings, to facilitate referrals in support of clients' treatment plan goals (i.e., psycho-social-medical services; HIV and TB testing and counseling; assistance obtaining driver's license, birth certificates, and Social Security cards; medication-assisted treatment; supportive housing, insurance). 
  • Provide recovery support and care coordination to enhance treatment completion including engagement, outreach, clean and sober socialization, life skills workshops, and other groups as required. 
  • Coordinate care with physical health, community health clinics and providers, and mental health providers,  
  • Engage clients in his/her treatment and recovery by identifying and developing additional supports such as linkages to self-help groups, peer support activities, and recovery monitoring and coaching. 
  • Advocate for clients across various systems to ensure appropriate and timely linkages to physical and mental health care, transportation, and retention in primary care services. 

EDUCATION AND EXPERIENCE:

  • Must be a certified Drug and Alcohol Counselor in compliance with all DHCS and BBS standards.

  • 2 years minimum of behavioral health experience.

  • Strong understanding of cultural competency with the target population.

  • Medi-Cal experience preferred.

  • Computer literacy necessary.
