LPN I (Finance)
Must be licensed with the State of Minnesota as a LPN, and must maintain current and unencumbered licensure.
Must be able to lift 20lbs, Stand, Sit, or Walk for long periods.
Must have reliable transportation to and from work.
Support the company values and maintain a values score of 4+.
Must be able to pass a criminal and/or abuse background check, and drug screening.
Must also meet at least 2 of the following criteria:
5+ years' experience working as an LPN in a direct care heath setting and/or
Previous experience supervising/team lead and/or
Previous experience working with tube feedings, ventilators, tracheostomy and/or
Previous experience working with mental health and/or
Previous experience leading team trainings and training unlicensed staff.
OTHER SPECIALIZED KNOWLEDGE AND ABILITIES:
Possess a sensitivity and aptitude for working with the elderly.
Possess skills to communicate effectively with those you supervise, residents, family members and RN Supervisor.
Possess leadership / supervisory skills.
Must be able to prioritize and organize work effectively and efficiently.
Able to work irregular hours, occasional weekends and evenings.
Ability to lead trainings as delegated by the Director of Nursing.
ESSENTIAL JOB FUNCTIONS AND TASKS:
Job Function: Work within Home Care program guidelines including completing essential paperwork and complying with state regulations.
Tasks:
Works to assure facility is in compliance with federal state and local standards and regulations. Is available for license surveys.
Maintains an up-to-date knowledge of current trends of Home Care.
Works to maintain privacy and confidentiality of resident, resident's records, and resident living environments.
Is willing to carry a cell phone and be available for staffing and notifications.
Communicates and documents resident data to staff as needed.
Assures residents maintain residency requirements, monitors and recommend alternative placement if necessary.
Reviews resident service agreements with RN, as needed.
Monitors and maintains resident charts and nursing notes.
Communicates any changes in resident's needs or conditions to physicians, as needed.
Monitors and follows through with ordering medications and all medication renewals as well as any necessary follow-up with physicians regarding medications.
Checks new medications from the pharmacy for medication administration by unlicensed staff.
Communicates in staff communication book any information that staff needs to be aware of.
