Full-time Case Manager position opening at our Short-term Residential Therapeutic Program (STRTP). Requirements: Bachelor's degree in Psychology, Social Work, Counseling or related field required. Sound personal qualifications. Four years experience in working with children in therapeutic setting. One year case management experience preferred. Must be 21 years old or over. A valid California drivers license with less than two minor moving violations is required of staff who have driving duties to meet the requirements of their position and be approved by the agencys insurance company. Responsibilities: Case Management Case management for children in the program under direct supervision of the program coordinator.Collaborate on all therapeutic interventions for clients at facility.Development and maintenance of an on-going treatment plan for each child.Maintain frequent and consistent contact with all members of the treatment team.Maintain appropriate documentation (case notes, progress notes and treatment plans/quarterlies).Coordinate contact with family members and other relationships outside the STRTP.Coordinate all aspects of educational planning. Meet with school personnel prior to enrollment of client. Attend all school meetings such as student study teams and IEPs.Participate in Child and Family Team (CFT) meetings in accordance with applicable laws and regulations and under direct supervision of program coordinator.Participate in referral process, intakes and discharges of assigned clients.Assess referrals for admission conjunction with the clinical team.Coordinate discharge planning from intake.Collaborate regularly with specialized therapists for each of the clients that have been referred for services.Coordinate discharge planning under direct supervision of program coordinator.Coordinate psychiatric, medical, medication, educational and other assessment/services with family and other providers.Collaborate with house management team to ensure all necessary forms for medication are on file and up to date.Assist with preparation of psychological evaluation requests when child is referred for clinical evaluation. Reporting/Emergency Response Maintain a moral, ethical and legal environment. Report misconduct that results in a policy violation or a violation of the law on any SVCFS site to immediate supervisor.Report and assess any event that may impact health, safety and welfare of clients including issues of suicidal or homicidal intent, injury, illness, medication error, safety, emergency and child abuse, verbally and with internal incident report form.Call and file unusual incident reports to the state for licensing issues.Ensure a written report has been submitted to child protective services for suspected abuse issues. Facility/Program Monitor all treatment interventions of the STRTP environment in conjunction with House Manager to ensure compliance with agency standards, Title 22, and Joint Commission standards and regulations.Monitor the environment, which directly affects the health and safety of clients. Report issues immediately to the program coordinator. Other: Attend all required meetings. Report any suspected child or dependent adult/elder abuse or neglect immediately to direct supervisor or utilize the chain of command if supervisor is unavailable. Report any client imminent danger to self or to others or gravely disabled immediately to direct supervisor or utilize the chain of command if supervisor is unavailable. Other duties as assigned.
