Partners in Care is seeking a full time Home Health RN Case Manager in Bend and LaPine, Oregon.
You will coordinate, implement, and evaluate home care services for our patients. Partners In Care is the oldest and finest hospice and home health organization in Central Oregon, emphasizing respect, compassion, and support within a family centered, full-service program. Partners In Care provides post-acute care to residents throughout a 10,000 square mile region of Central Oregon who are medically fragile, experiencing chronic and serious illness, recovering from surgery, or need end-of-life care at home or in our hospice in-patient facility called Hospice House. As an independent non-profit organization, Partners In Care works in close partnership with the patient or clients family, the community, and other care organizations to ensure quality care.
Job Responsibilities
The Home Health RN Case Manager completes an initial comprehensive (includes OASIS) and accurate assessment of patient and family to determine home health needs. Provides a complete physical assessment and history of previous illness (es) and uses the health assessment data to determine appropriate diagnosis and need for other disciplines and resources. Develops an individualized care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Makes necessary revisions to the care plan as patient status and needs change. Completes and updates the ongoing assessments, and care plan as patients status changes. Provides health care instructions and education to the patient and or family as appropriate per assessment plan and patients caregivers ability to learn. Adapts teaching styles to meet patients needs and familys needs.
Applies nursing principles and techniques in the treatment of patients. Provides nursing care, emotional support, and education to the patient/family/ caregiver. Administers medications and treatments as prescribed by the physician. Ensures that arrangements for equipment and other necessary items and services are available. Provides health care instructions and education to the patient and or family as appropriate per assessment plan and patients caregivers ability to learn. Adapts teaching styles to meet patients and familys needs. Communicates with all members of the healthcare team to promote coordinated, efficient care and documents coordination of care with all involved with patient care.
