Grief Counselor/MSW (Finance)
The grief counselor is responsible for assisting in planning, implementing, and maintaining all aspects of the Partners In Care bereavement program.

Partners In Care is the oldest and finest hospice and home health organization in Central Oregon, emphasizing respect, compassion, and support within a family centered, full-service program. Partners In Care provides post-acute care to residents throughout a 10,000 square mile region of Central Oregon who are medically fragile, experiencing chronic and serious illness, recovering from surgery, or need end-of-life care at home or in our hospice in-patient facility called Hospice House. As an independent non-profit organization, Partners In Care works in close partnership with the patient or clients family, the community, and other care organizations to ensure quality care.
Job Responsibilities
Coordinates and conducts the bereavement assessment process.
Provides grief counseling, education, and support services to hospice families/caregivers per established protocols.
Coordinates and develops a bereavement plan of care to meet the identified needs of the bereaved.
Provides bereavement support to organization staff coping with grief/loss issues as requested. Assists in planning/coordinating other services to meet staff support needs.
Provides bereavement information, referral, and support services to community members seeking assistance.
Functions as part of the Interdisciplinary Care Team coordinating/consulting with other team members to ensure timely and appropriate delivery of bereavement services/support both prior to and after a patients death.
Assists in training, assigning, supervising, and supporting bereavement volunteers.
