- Assist with admissions, discharges, and resident care plans
- Support residents and families in navigating care transitions
- Help coordinate appointments, community resources, and family communication
- Advocate for residents' rights, dignity, and emotional well-being
- Maintain documentation in accordance with regulations and facility policies
- Partner with the Social Services Director and interdisciplinary team to provide the best care possible
