RN (Weekends) (Healthcare)



Job Summary

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our nursing team in a Skilled Nursing Facility (SNF). The RN plays a critical leadership role in delivering high-quality, resident-centered care and supervising the care provided by Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs). This position ensures that each resident's physical, emotional, and psychosocial needs are met while upholding the highest standards of clinical care, safety, and regulatory compliance.

Key Responsibilities

Clinical Care & Oversight

  • Provide direct nursing care, including assessments, wound care, IV therapy, and other clinical interventions as prescribed.
  • Conduct comprehensive resident assessments and develop individualized care plans in collaboration with the interdisciplinary team.
  • Monitor residents' health conditions and respond promptly to changes in status.
  • Lead and supervise LPNs and CNAs to ensure consistent and appropriate care delivery.
  • Coordinate admissions, transfers, and discharges, ensuring accurate documentation and continuity of care.

Medication Administration & Documentation
  • Administer medications and treatments according to physician orders and facility protocols.
  • Maintain detailed and accurate medical records, including documentation of care provided, assessments, and resident responses.
  • Ensure proper storage, administration, and documentation of controlled substances and medications.

Leadership & Communication
  • Communicate effectively with residents, families, and healthcare professionals to support coordinated care.
  • Provide clear direction and support to LPNs, CNAs, and other team members during shifts.
  • Participate in interdisciplinary care meetings and support family care conferences as needed.

Compliance & Quality Assurance
  • Ensure nursing care is provided in accordance with state and federal regulations and facility policies.
  • Assist in audits, surveys, and inspections to ensure continued compliance and quality of care.
  • Promote and participate in quality improvement initiatives and resident satisfaction efforts.

Professional Development & Team Collaboration
  • Maintain active RN licensure and complete continuing education as required.
  • Provide training and mentorship to nursing staff and participate in orientation of new employees.
  • Attend staff meetings, in-services, and development programs to stay current with nursing standards and practices.

Safety & Infection Control
  • Adhere strictly to infection control protocols and universal precautions.
  • Promote a safe environment for residents, visitors, and staff, and report any safety hazards promptly.
  • Respond effectively to medical emergencies and participate in emergency preparedness activities.
Qualifications

Licensure & Certifications:

  • Valid and current Registered Nurse (RN) license in the state of Oregon.
  • CPR and First Aid certification (required or obtainable upon hire).

Experience:
  • Prior experience in a skilled nursing or long-term care setting strongly preferred.
  • Familiarity with PointClickCare (PCC) or similar EHR system is a plus.

  • Ability to lead and support a team in a fast-paced, dynamic environment.
  • Compassionate and resident-focused approach to care.
  • Basic computer proficiency required.

Physical Requirements:
  • Ability to stand, walk, and sit for extended periods.
  • Ability to lift/move up to 50 lbs.
  • Ability to respond quickly and appropriately in emergency situations.
