Job Summary
The School Health Nurse Clinician, Grant TCT School Health Program Nursing Coordinator provides oversight for planning, implementation and evaluation of the Grant TCT School Health program and ensures the systematic and coordinated delivery of school-based nursing services. The School Health Nurse Clinician is a community-based position embedded in a school health program operated by a pediatric hospital, serving 30 underserved schools across Miami-Dade County. This advanced nursing role provides clinical leadership, care coordination, and oversight to a team of school-based nurses to ensure the delivery of high-quality, equitable health services to students. The Nurse Clinician is responsible for supporting the implementation of comprehensive school health initiatives, including chronic disease management, mental health screenings, emergency care planning, and compliance with state and federal health mandates. The role involves conducting student assessments, mentoring nursing staff, developing individualized healthcare plans (IHPs), and collaborating with families, school personnel, and medical providers. With a focus on preventive care and improving access to services, the School Health Nurse Clinician plays a vital role in addressing health disparities, supporting student wellness, and ensuring that children are healthy, safe, and ready to learn.
