Occupational Therapist - Home Health (Healthcare)



The position location is Peoria and surrounding communities working with our Carle Health Methodist Home Health Team.

Position Purpose
As a member of the therapy team, the Occupational Therapists primary function is to provide a thorough therapy evaluation, a detailed, plan of care including treatment methods and procedures; supervision and guidance of Assistants and consultation with referral sources and other healthcare providers.

Requirements

Qualifications
Licensed or eligible for licensure by the State of Illinois under the Illinois Occupational Therapy Practice Act.Member or eligible for membership in the AOTA preferred.General knowledge and skill enabling the evaluation and treatment of a wide variety of clients/patients such as neonates, pediatrics, young adults, adults and/or geriatrics.Current certification in CPR and other service line specific competencies.
Essential Job Functions
Perform client-centered evaluations and establish an Occupational Therapy treatment plan.Provide evidenced-based Occupational Therapy treatment.Case management functions: evaluate effectiveness, revise the plan, and discharge from services.
