MAP Navigator (487) (Transportation)

The MAP Navigator is responsible for engaging individuals experiencing homelessness or other barriers by administering the Community Screening Tool and VI-SPDAT, developing individualized linkage plans, and arranging warm hand-offs to appropriate services. The Navigator provides case management through the MAP system, collaborates with community providers and faith-based partners, and advocates on behalf of clients to support their transition from crisis to long-term wellness and recovery.


How will I contribute

Administer the Community Screening Tool and VI-SPDAT to assess client needs and determine service linkages.
Develop individualized linkage plans and ensure warm hand-offs to appropriate services, including housing, healthcare, employment, and community supports.
Provide advocacy and support to assist clients in meeting basic needs such as food, shelter, clothing, and documentation.
Monitor client progress throughout the linkage process; identify and address barriers to accessing services.
Utilize best practices to build rapport and engage clients, supporting their journey from crisis and hopelessness toward stability and sustainability.
Provide case management for individuals experiencing severe mental illness, co-occurring disorders, physical illness, unemployment, or homelessness.
Cultivate and maintain strong working relationships with service providers, treatment programs, community-based organizations, county agencies, and faith-based entities.
Demonstrate an understanding of the complexities of working with clients who have co-occurring conditions, exercising patience and empathy.
Maintain knowledge of community resources, including housing, legal services, healthcare, addiction recovery, and natural supports.
Document all client interactions, linkages, and progress in the MAP system accurately and timely.
Perform other duties as assigned to support the overall functioning and success of the program.
Use of company vehicles may be subject to monitoring via in-vehicle dashcam systems for safety, compliance, and operational purposes. These systems may collect video, audio (in limited situations), and telematics data during vehicle use. As a condition of employment in a position requiring the operation of agency vehicles, employees are required to acknowledge and consent to the use of such monitoring technologies. Refusal to provide consent may disqualify an individual from being assigned driving responsibilities. All monitoring is conducted in compliance with applicable California privacy laws.
May encounter risk of working with persons with SMI (serious mental illness) that may include, but not limited to persons in mental health crisis, persons that may be intoxicated or impaired by drugs or alcohol, persons who may be DTO (danger to others), DTS (Danger to Self), or GD (Gravely Disabled). If such situations arise, staff will be required to call local emergency personnel to help deescalate the crisis, use crisis intervention techniques, and consult with supervisor as soon as practically possible.



What I bring

Bachelor's Degree: Social Work, Psychology, Human Services or a related field
Class C Drivers License
2 years experience in case management, client linkage, or related services; or comparable quantitative field or equivalent related work experience. Experience working with individuals experiencing homelessness, severe mental illness, co-occurring disorders, or other significant barriers.
Knowledge of community resources, housing systems, and support services required. Must have a strong understanding of community-based resources, housing systems, and supportive services for individuals experiencing homelessness, mental illness, substance use, and co-occurring conditions. Knowledge of best practices in case management, trauma-informed care, and engagement strategies is essential. Familiarity with the VI-SPDAT and Community Screening Tool, as well as the ability to build and sustain partnerships with treatment providers, community organizations, county agencies, and faith-based supports, is required.
Must be able to engage and build trust with clients who may be in crisis, demonstrating empathy, patience, and cultural sensitivity. Ability to identify barriers, problem-solve effectively, and advocate for clients to access needed services is essential. Strong communication, collaboration, and organizational skills are required, along with the ability to maintain accurate documentation and manage multiple priorities in a fast-paced environment.
Demonstrate compassion, patience, and resilience when working with individuals facing complex challenges. A strong sense of integrity, professionalism, and cultural humility is essential, along with the ability to remain calm and solution-focused under pressure. Flexibility, adaptability, and a genuine commitment to client advocacy are key characteristics for success in this role.
Skills in motivational interviewing, trauma-informed care, and crisis intervention are highly valued. Experience collaborating with multidisciplinary teams, navigating community resources, and building partnerships with faith-based and community organizations is also preferred.
