Division/Department/Program: Assertive Community Treatment (ACT)


Assertive Community Treatment (ACT) uses a multidisciplinary team approach to provide intensive mental health treatment services to individuals with severe and persistent mental illness in their homes and other community settings-24 hours a day, seven days a week, 365 days a year. ACT is often referred to as a "hospital without walls" and its goal is to keep individuals out of state hospitals, jails, emergency rooms and other institutions. ACT clients must meet a strict criterion (i.e., diagnosed with a severe and persistent mental illness, unable to maintain activities of daily living, and be unsuccessful in other forms of treatment). ACT clients often have no resources and no family support and require an intensive and expeditious form of treatment.

Supervises, guides, and/or instructs the work assignments of subordinate staff. Supervises and directs the clinical activities of a comprehensive behavioral health program. Performs psychosocial assessments, treatment planning/monitoring and discharge planning. Provides counseling/therapy to clients requiring complex intervention. Performs commitment determinations and/or executes orders of involuntary commitments.

Duties& Responsibilities:

Directs the day-to-day clinical operation of the ACT team.
Lead the daily organizational staff meetings and treatment planning meetings.
Schedule staff work hours to ensure appropriate coverage for day, evening, weekend and holiday shifts and on-call hours.
Direct and coordinate the client admission process and treatment rehabilitation, and support services of the program in coordination with the psychiatrist.
Provide clinical supervision for each client, on the development of comprehensive assessment and the treatment plan.
Participate in staff recruitment, interviewing, and hiring. Provide leadership in team development and team building activities to promote staff morale, retention and intercultural competency.
Provide service coordination to ensure quality care. Develop and administer ACT program budget. Supervise medical record management.
Carry out and document quality assurance activities and review of use of program services.
Develop, maintain and revise as necessary, program policy and procedures. Participate in providing treatment and dual diagnosis services.
Assist the Director in meting program and organizational needs.
Other related duties as assigned.


Minimum Qualifications:
Master's degree in social work and counseling from an accredited college or university and licensure in the area of assignment and three (3) years of experience at the lower level Social Worker, Licensed 3 (HCP052) or position equivalent; or Master's degree in social work and counseling from an accredited college or university and licensure in the area of assignment and three (3) years of experience in the provision of therapeutic services with, which includes one (1) year in a managerial or supervisory role.

Note: Some positions may require licensures such as an LAPC, LPC, LMSW, LCSW, LAMFT, or LMFT (associate or full licensed required)

Preferred Qualifications:

Licensed as LPC or LCSW in the state of Georgia
Minimum of two (2) years' experience in providing substance abuse and mental health treatment to individuals in the community, doing in-home visits.
Must have a clear understanding of characteristics of problems of adults with severe and persistent mental illnesses.
Supervisory and program management experience is desirable.
Experience in using Carelogic computer system.


Requirements/Competencies:

Must be able to lift 20 pounds.
Requires long periods of sitting/standing.
Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
Candidates for selection must pass a criminal background check (including fingerprinting).
Must pass a pre-employment drug screen and subject to random drug screens.
Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities
