Case Manager - Hem Onc (Healthcare)
Children's Hospital Los Angeles (CHLA) is a 501(c)(3) nonprofit institution that provides pediatric health care and helps our patients more than half a million times each year in a setting designed just for their needs. Our history began in 1901 in a small house on the corner of Alpine and Castelar Streets (now Hill St. in Chinatown) and today our medical experts offer more than 350 pediatric specialty programs and services to meet the needs of our patients.
Position Summary: The Nurse Care Manager performs complex discharge planning for patients of different ages and developmental levels with serious illnesses. Focuses on patients with complexities related to finance and/or resource utilization. Acts as advocate for the patient/family by collaborating with the multidisciplinary team to develop and implement a case management plan to ensure quality patient outcomes, and customer satisfaction in a cost effective manner. Participates/facilitates in the development and outcome analysis of clinical pathways as a member of the multidisciplinary CQI team.
Minimum Qualifications: 3+ years advanced level pediatric nursing experience in Oncology (Leukemia/Lymphoma, Chemotherapy) with utilization management and/or discharge planning experience. Thorough knowledge of managed care, utilization management, discharges planning, continuous quality improvement concepts and willingness to learn. Computer Literacy. Strong communication, interpersonal, organizational and negotiation skills required. Fluency in both English and Spanish desired.
Education/Licensure/Certifications: Bachelor's Degree in Nursing required. Master's degree in Nursing, Health Care Administration, or related field preferred. Current Registered Nurse licensure in the State of California and AHA CPR/BLS certificate required.
