Medical Records Specialist - Disability Forms (Healthcare)



Job Summary

Responsible for completing disability forms in a timely and efficient manner, ensuring accuracy and providing our customers with a high quality product and positive, patient-centered experience.  Experience with FMLA and disability form preparation required.  Hybrid work environment:  Combination of remote and in-office environment.     

Duties and Responsibilities

  • Process FMLA/disability forms as requested by patients, their employers or ins. carriers. 
  • Communicate with patients, insurance carriers and providers to obtain the needed information to accurately complete the form.
  • Prepare a Medical Records Forms Receipt for each form…capturing fax and addresses to accurately deliver the information to the appropriate party.    
  • Answer/educate patients about the receipt of the form, fee required for form preparation and   process payment as needed.  
  • Complete disability forms after payment within 10 business days.  
  • Verify patient information using two-patient identifiers.
  • Communicate with patients on the status of their forms via portal or phone.  
  • Process all forms with proper adherence to HIPAA and HITECH compliance training and laws.
  • Answer incoming calls related to forms.  Transcribe voicemails into EMR as needed.  
  • Establish and maintain professional relationships with all team members and customers.   

Requirements and Qualifications

  • Must have excellent communication skills.
  • 1 year prior experience in a medical records department or like setting.
  • Strong computer skills, general working knowledge of Microsoft Word, Excel and Medent EMR preferred.
  • Must be able to type 50 wpm.
  • Must be able to use fax, copier and scanners.  
  • •    Must be self-motivated, a team player and proven customer satisfaction skills.  

Physical Demands

  • Position requires manual and finger dexterity and hand-eye coordination.
  • Involves standing, sitting, and walking.
  • Team member will occasionally be asked to lift and carry items weighing up to 10 pounds; normal visual acuity and hearing are required.
