What We Need From You
Barnes Healthcare Services is seeking a Clinical Transitional Liaison (CTL) that will be assigned to one or more facilities in the Atlanta sales territory. The CTL supports the sales efforts of the Territory Manager (TM) by working in collaboration with case management and social work staff at the assigned facility. The expectation for this position is that the CTL will travel daily to the facility or facilities to develop relationships with discharge planners and case managers and review the needs of the patients being referred to the home setting or requiring ambulatory infusion care. The CTL provides continuous evaluation of the effectiveness of treatment through ongoing assessment of the patient. The CTL provides the therapy to teach, train, and instruct in the hospital prior to discharge to the home setting. The CTL communicates timely with the discharge planner and/or social worker to coordinate the needed care for patients at home. Effectively communicates with sales and operations teams regarding patient needs and any issues with the facility to which they are assigned.
