Care Team Nurse (Healthcare)



This is a fast-paced position providing support for assigned providers while managing a high volume of patient calls into our clinical department. The ability to prioritize and multi-task efficiently is a must. Attention to detail is key in this position. As a nurse, you will work collaboratively with our providers and clinical staff as well as other department staff to provide high quality care for our patients. This is a full-time position, Monday-Friday 8am-4:30pm.
Must have one year of experience in a hospital or clinical setting. RN license is required.

Position Description:

The Nurse prepares medication refills and patient messages to help optimize the use of provider time and resources.
The Nurse will use a variety of communication tools and systems to handle medical information requests and updates, communicate test results at the direction of the provider, and work collaboratively with the rest of the clinical department fielding incoming phone calls.
As a member of the clinical department, the Nurse will maintain complete, timely, and accurate documentation for each patient and continually look for opportunities to improve processes and workflow.

Major Areas of Responsibility:

Answering phone calls, taking and delivering messages to providers and patients by phone, verbally, or electronically. This will involve obtaining detailed and complete information according to the patient's diagnosis and medical issue.
Preparing medication prescriptions for the provider to review and sign either physically or electronically. This will involve chart review for overall patient compliance, checking: labs, appointments, calculating medication use and dosing for compliance
Communicating issues with medication /prescriptions with pharmacy staff over the phone when necessary.
Providing individualized patient care and maintaining clinical competency appropriate for neurology practice.
Maintaining close communication with providers and other care team specialists.
Preparing all necessary paperwork in a timely manner including but not limited to; prior authorizations, FLMA forms, work ability forms, orders/referrals, and work notes.
Exhibiting a positive customer service attitude.
Continuously improving work processes, environment, and overall experience to serve patients and meet clinic goals.
Following and adhering to guidelines for proper medical chart documentation
Maintaining nursing license license and practicing within the proper scope of nursing according to the Minnesota Board of Nursing (AAMA).
Update medication list and chart maintenance according to provider protocol
